That's how it is done guys! This is how some professionals do their touchup in the studio.
1) Map out your steps in retouching via an overlay layer where you put down scribbles, notes etc. This will ensure you won't miss out the important areas in your digital touch-ups later.
2) Note that small blemishes in the skin, clothing are touch-upped in a painstakingly way, bit by bit. That's how it is done.
I did my share freelancing for a photo studio; retouching portrait shots, and believe me, the money is very hard-earned. 8 SGD for retouching a whole pack (3-4 photos) which can take up to 2-3 hours.
3) Note that "slimming down" models in photo shoots are very common; some digital make-up will be applied too!
4) Professional tools: A iMAC computer and Wacom Intuos Tablet!
